2010-2011 Application Instructions

Please fill out this form in order to apply for enrollment at Open High School of Utah for the 2010-2011 school year.

Only one application is needed per family.

I understand this lottery application is good for one school year only. If my child is not accepted into this school, I must reapply during the first Open Enrollment for the next school year.
